Registration

All patients wishing to register will be requested to prove their identity and address. If you are a registered patient with the NHS please bring your medical card to the Surgery. If you have not previously been registered under the NHS you will be asked a number of questions to satisfy the patient status guidelines. 

When registering you will be asked to complete a form about your health (available below) and also a NHS Registration form. We need all new patients to have a New Patient Health Screening to find out their medical history. If you have a preference for a particular practitioner we shall notate this in your medical notes.

Please note that registration of new patients is between 8.30 am - 5.30 pm weekdays.
